Held in South Tyrol, an autonomous province in the far north of Italy where most residents are native speakers of German, the Stilfserjoch Stelvio Trail Run (until 2022: Stelvio Marathon) is an extremely demanding uphill trail race for super-fit mountain runners and hikers.

The route is 21km long and includes 2,100m of ascent, starting in the village of Prad am Stilfserjoch (Italian: Prato allo Stelvio) which sits at an elevation of 915 metres in the Vinschgau (Val Venosta) valley.

The finish line is at 2,845 metres above sea level at the mountain hut on the Dreisprachenspitze (Cima Garibaldi), 87 metres above the Stilfserjoch (Passo dello Stelvio), the second-highest paved mountain pass in the Alps (2,758m).

Located on the border between Italy and Switzerland, the Stelvio Pass has featured 13 times in the Giro d'Italia bicycle race. Unlike the cyclists, the runners don't use the spectacular Stelvio Pass Panorama Road with its 45 hairpin turns (pictured). The race is run almost entirely on trails and gravel roads high above the mountain pass road.
